We are open Monday through Friday from 8:30 a.m. until 5:00 p.m. and on Saturdays from 9:00 a.m. until Noon. To schedule an appointment, call 206-987-7210.
If this is your first time visiting our clinic, we will need to register your child before we can make an appointment. You may click here to open a registration form. Please print it out and then mail, fax, or walk it in to the clinic. Our fax number is 206-987-7206. You may also contact us and we will be happy to mail or fax you the forms.
