We are open Monday through Friday from 8:30 a.m. until 5:00 p.m. and on Saturdays from 9:00 a.m. until Noon. To schedule an appointment, call 206-987-7210.
If this is your first time visiting our clinic, we will need to register your child before we can make an appointment. You may click here to open a registration form. Please print it out and then mail, fax, or walk it in to the clinic. You may also contact us and we will be happy to mail or fax you the forms.
