If this is your first time visiting our clinic, we will need to register your child before we can make an appointment.  You may click here to open a registration form.  Please print it out and then mail, fax, or walk it in to the clinic.  Our administrative fax number is 206-987-7206. You may also contact us and we will be happy to mail or fax you the forms.

If you want to make a first-time mental health appointment, we will need a referral from your child's primary care provider.  You will also need to fill out an intake packet. To have one sent to you, please call 206-987-7260.
